INDIAN CULTURAL ASSOCIATION OF MINNESOTA (ICAM)
This Olmsted County Community Health Needs Assessment (CHNA) community survey provides valuable information that helps our community define and understand the most pressing health priorities in our area. The survey also informs collaborative initiatives to improve health for all in our community.
It will take roughly 20 minutes to complete the survey. It asks questions about factors that affect our health, including clinical care, the physical environment, and personal behaviors. Participation in the survey is completely voluntary. Your answers are strictly anonymous; no identifying information will ever be linked to any of the responses.
The Olmsted County CHNA is a collaborative effort that began in 2012. Olmsted County Public Health Services, Olmsted Medical Center, and Mayo Clinic lead this effort to keep our community healthy, supported by many community organizations.

There are many ways to get tested for COVID-19 in Minnesota.
State-run, low-barrier community testing sites: These sites are located throughout the state and offer saliva PCR tests, nasal-swab rapid tests, or both. You should have PCR test results in about 24-48 hours after the lab receives your test. Rapid test results should be provided within 15 min-3 hours. State community test sites are always free.
Pharmacies: Find pharmacy testing locations on our website, or call ahead to see if they have on-site or drive-thru testing.
Free at-home saliva PCR tests provided by the state: Test kits are sent to you directly. You take the test at home over Zoom, and drop off your test kit at a pick-up location. You should have test results in about 24-48 hours after the lab receives your test.
Pop-up testing: These are testing events held by local public health, tribal health, workplaces, community organizations, or other partners.
Over-the-counter self-tests: These may be available online or at a store or pharmacy.
Clinics: Call ahead to make sure they can test you. Some clinics may require an office visit, which may include a co-pay or fee.
